It turned out the Java library I was using to create the pdfs were not
compliant with GhostScript 7.05.
A minor fix by the PDF Java library vendor solved this issue.
Many thanks to Lee for setting me in the right track.

typerules is only utilized for sendfax.  It pre-processes the document 
before submission to hfaxd.

I recommend that you look into upgrading/fixing Ghostscript.
