Where is it getting stuck?

I recently diagnosed similar symptoms, and checked maillog to discover
that the outbound mail wasn't even being attempted.  I then went to the
/var/spool/fax/bin directory and discovered that for unknown reasons, the
executable flag was missing on a number of key files.  chmod ugo+x fixed
this problem.
