i had the same sort of problem with this originally and found it was notepad
printing to a postscript driver.  bashing my head against the desk several
hours and finally on the off chance i tried it from word and it worked fine.

what i did to troubleshoot was to add:

LOGFILE=/var/log/cups/fax.log

to the sambafax and then every place where i wanted to watch the status of
the script i simply echoed something to the $LOGFILE:  
----
echo "$TODAY - Start Fax from: $MailTo" >> $LOGFILE
echo "$TODAY - Bad Number : $FAXNUM - Sent Mail to $MailTo " >> $LOGFILE
----


this may help you pin down where it's falling over...

If i try to send a fax without the 
Fax-Nr  :  0541-7363556
An e-mail come back to me to inform ecc ecc .
But if i try to send a fax with the number nothing appear. It seems to be
hylafax not dialogue with cups. I haven't make nothing to hyalafx it is
installed by default in /var/spool/hylafax.
Of course if i try to send a fax from a windows client nothing appear.

Ther is someone that can help me please
