The procedure, as documented, is to create a coverpage using Tgif with
the desired variables designated as XXXX-variable.

Note that the XXXX is all uppercase, not lowercase.  This may make a 
difference in the outcome, but I haven't tried it.

Next, the cover page is saved from Tgif as an encapsulated PostScript (eps)
file.

Then this eps file is processed with make_faxcover to generate a PostScript
template that can be used to produce the final coverpage with faxcover.

Also, what is _not_ documented is that the template file generated with
make_faxcover should be manually edited to remove the LAST LINE of the
file, which should be "showpage."  If this last line is not removed
the coverpages will be two pages in length with the second page blank.
