I do a similar thing already but via MS Word and mail-merge. I just print the merge doc to a postscript Samba printer set up on our OpenBSD box. A shell script and simple c (text parser) program sitting on the Unix box as a printer driver does all the work - parses the postscript document for a "Fax-To:" then a fax number. It gives you a message via sendmail if there is no fax number in your document.
