This usually means you didn't run faxaddmodem for the specified device.
Look in /var/spool/hylafax, you should see a file called FIFO and many files
called FIFO.<devid>, one for every modem device.

Why do you use "-D"? This is my line
S1:2345:respawn:/usr/sbin/faxgetty ttyS1
Is, by any chance, ttyD0 a link to cud0?
