HylaFAX bug.  Go back to the default config file for the USR modem (re-run
faxaddmodem, choose Class 1).  You'll need to apply the patch on:

http://bugs.hylafax.org/bugzilla/show_bug.cgi?id=130

recompile, reinstall, and retry it.
