My guess is that your config file isn't 100% right. To verify this, open up minicom (you'll have to disable the hylafax daemons) and send the command 'AT+FTH=3', and if you get (as I believe you will with a USR modem) 'ERROR'. I think that you're probably using a Class 1 config file, so I don't have any experience there to tell you what the correct setting is, though.
