Eesh... can't help you with the specific problem, but I *can* say that there have been numerous improvements and bug fixes with HylaFAX, and you should use something newer (i.e. use 4.1beta2). Also, I can say that the 3Com/USR modems (in particular Courier) haven't been the most robust of modems to use for faxing... although I don't use one and couldn't tell you if that's a valid interpretation or not.
