flexfax: Communication failure during Phase B/C

I'm getting an error message that I've never seen before I moved to a
different machine to run HylaFAX 4.0pl2 with a ZyXEL 1946E:

Communication failure during Phase B/C (modem protocol botch)

Looking at the log:

Did the remote hung up prematurely and piss off HylaFAX...?  Is this more a
remote problem, or a symptom of a local misconfiguration?
