Phase C errors with USR 33.6

Hi all. I'm using hylafax-4.0pl2 on RH6 with an external USR sportster
33.6 modem. It seems to be working ok, except for the occasional problem:
1. I receive the following messages occasionally:
Oct 6 13:40:43 fax FaxGetty[5353]: RECV FAX (00000095): session with 201
555 0160 terminated abnormally: Unspecified Phase C error, including too
much delay between TCF and +FDR command
Oct 6 13:40:43 fax FaxGetty[5353]: RECV FAX: bin/faxrcvd
"recvq/fax00063.tif" "ttyS0" "00000095" "Unspecified Phase C error,
including too much delay between TCF and +FDR command"
It seems to have received part of the fax, but not all of it. What could
be causing this? The remote end, perhaps?
2. Occasionally faxstat reports "waiting for modem to become ready" when
in fact there is no fax coming in. It seems to occasionally not hang up
after receiving a call. I need to kill faxgetty and getty, at which point
it works again.
