The "Sender:" line refers to the sender's TSID, which apparently was not sent by the sender or it was simply an empty string. The "NMBR=" line refers to the sender's CallerID, and the reason it's not displayed in your faxinfo output (which is in your notification e-mail) is due to the version of HylaFAX that you are using.
