You have two glaring problems:

1. You have a USB modem. They are 'soft modems' and will never work as well as a real UART based modem connected via serial or an internal hardware modem. Be prepared for odd issues that seem unresolvable.

2. You have a USRobotics modem. USR modems are notoriously unreliable with fax. See the list archives for the array of issues you may experience.

Other than that, please provide some additional information about what problems you're having. So far, your description hovers somewhere between vague and non-existent.
