Your modem is "wedged" (locked-up, unresponsive).

It's either a problem with the modem itself or the serial controller.  
When you reboot to fix the problem are you also power-cycling the 
modem?  If not, then it would seem to be a problem with the serial 
controller (or the Linux serial driver, I suppose).
