It seems they are tiping twice destination fax number. The sender fax
machine is a Toshiba 281C and the problem happens every time from this
number.

Moreover, the tag line changes in the PDF file: if they send a 40 pages
document and a few minutes later a second 5 pages document, and both
faxes are batched in one batch, I see two different dates in the tag
line while browsing the single PDF i receive.
