Fax header info:

In file /var/spool/hylafax/etc/config.tty??? tweak the following lines:

...
LocalIdentifier:        "Fax Header Text"
TagLineFont:            etc/lutRS18.pcf
TagLineFormat:          "From %%l|%c|Page %%P of %%T"
...

%%l is the LocalIdentifier

I added the below lines you mention in the master.cf file of postfix and
still the header is sent.  No cover page is being sent.

Below you have ax		unix ...
Should that be fax	unix ...
?

The switches on faxmail remove the header page (you also need to turn
off 
the cover page).

For postfix, then, the options would be:

ax       unix  -       n       n       -       1       pipe
   flags= user=fax argv=/usr/bin/faxmail -d -N -T -n ${user}
