Are you able to talk to the modem via cu or minicom? It's possible that 
HalaFAX would support your modem. I'm using a Zoom 56K internal modem.

I'm considering to install the Hylafax on a new SUSE (linux) server.
I have an old external Data\Fax Modem -  MultiModem , MultiTech
MT5600ZDX.
It connect to the serial port of the server.
Will the Hylafax support it?
