First of all, I would recommend against using USR modems in Class 2.0.  
Re-run faxaddmodem and choose Class 1 with default flow control.

The fact that the modem didn't respond to the AT+FKS command at all 
(either with an OK, ERROR, or something... it would indicate that the 
modem had crashed... and it will be a number of seconds before it will 
return to working order... although the fax session would have failed.

This kind of thing is not at all uncommon with USR modems.  It's one 
reason why we've been advising against them for many years now.  
However, switching to Class 1 will give you the greatest chance of 
success with the modem.
