If this is a software ("Win/Lin") modem then I would suggest that you take a look at http://www.linmodems.org/ which will help you discover whether a Linux driver is available.  That said, if it is a software modem then you will not have a great deal of joy with fax performance or reliability.
 
If this is a hardware modem then you should consult the manufacturers website for suitable drivers or instructions.  Again, the performance and reliability will depend on whether the modem manufacturer cares about fax.
