By "turning up the logging" I presume you mean making "messages" contain more info than normal? Rats. I'll have to look that one up.
Mean time, what I see is like this:

Well, it should be working, then. Now you get to try to troubleshoot. :-) Start by turning up the logging, noticing the notify command there, and then running notify by hand from the /var/spool/hylafax/ directory... to hopefully get some errors being mentioned.

Does FaxDispatch need the same thing? It has FILETYPE=PDF;FILETYPE=pdf
Again, case is important.
FaxDispatch is for incoming faxes and FaxNotify is for outbound.
