On error you'll still get a duplicate.  If SENDTO=faxmaster@localhost 
then you'll want NOTIFY_FAXMASTER=never, because SENDTO will be getting 
errors as well.

NOTIFY_FAXMASTER usually does go in FaxDispatch.  Check your FaxDispatch 
file to ensure that there's not a syntax error or something that is 
causing it to exit before setting NOTIFY_FAXMASTER.
