I've just looked at that exact modem thanks to another post on this list
(I should have read it before making my request).

MultiModemZPX PCI-U-NV Data/Fax  [MT5634ZPX-PCI-U-NV] - $79.00

We have two offices each with their own fax number.  I've been trialing
hylafax on the less busy number with a debian system and that el-cheapo
USR modem.  I'm impressed with the application, and the staff are loving
the PDF email attachments, so I think I'll purchase one of the
MultiModemZPX cards for the busy site and will continue to monitor the
performance of this USR modem on the quieter site.  If it continues to
perform poorly I'll replace it with a MultiModemZPX.

The MultiTech modems are below $100 -- typically around $80 or so -- and

the MultiModemZPX PCI-U-NV (the V.92 version) units have worked very 
well in our testing.

And yes, USR modems *are* that bad... they're what we were trying to use

before we switched to MultiTech, and we had *lots* of pages dropped 
partway through and other miscellanious bad stuff, all of which has gone

away with the switch to MultiTech hardware.
