The client program can specify which modem to use.  The client program 
also produces the fax cover sheet, so it can do whatever it pleases in 
that respect.  However, in both cases you must be able to trust the 
client (user) to provide accurate information here.

it was suggested that we have a type of "SourceControls" that would 
allow the server to set various things based on the "owner" or submitter 
information without particularly trusting the client settings.  Nothing 
became of that, however.  (Although, I do still know of some people that 
would like that feature.)

Cover sheet generation is usually done client-side.  Although there is 
some work done to make server-side cover sheet generation available, I'm 
not sure if it's usable or not yet.
