Exit status 0xb is a segfault.  You should probably increase 
ServerTracing in /var/spool/hylafax/etc/config to 0xFFF, take note of 
the conversion script command, and then run that by hand from the 
/var/spool/hylafax directory.  Hopefully that will give some clues.  If 
not, then post that information and results here.

It could be libtiff or ghostscript.
