Wow, another Davicom modem.  I had a 33.6 Davicom modem once (maybe I 
still do in my drawer full of ISA modems), but it only did Class 2.  I'm 
sure that the chipset was different.

Is the chipset on the modem also Davicom?

When you run faxaddmodem it's not going to pick up a prototype so you're 
going to need to make your own.

Anyway, as far as the blacklisting, you'll need to read the modem manual 
about how to manipulate that.
