Figured out the command init -q to reload the inittab settings.  Now it
sees the call coming in.

Could be, I have changed to /dev/ttyS0 instead of /dev/modem, and now I
can see faxstat picking up something in /var/log/messages

Now I see this:

Just to clarify, this setup did work before reloading the box, so unless
something broke coincidently in the last week or so, the hardware is OK
