This is a very common USR firmware error. I had thought that maybe the
presence of the +FRS/+FTS commands was the catalyst for this, but you
appear to have tested it...

So, it looks to me like you're stuck in a rut with that modem.
Does this look indeed to be the modem, or is there something else that
I can
try changing?
Most certainly looks like USR garbage, yes.
