I used excel to duplicate one of our pre-printed forms. Then I use the Adobe
generic postscript printer driver, and print it to a .ps file. I then use
ImageMagik to convert this .ps file to an .eps file, and the blank form is
done.  I use enscript and the "epsf" function it provides (see the enscript
man page) to overlay the text file onto the eps form. It outputs a postcript
file, which I then pass to sendfax for sending.  It works perfectly, other
than the pesky "pages to follow" problem.  :-)
