This is a Multi-Tech 2834ZDX on ebay.  It is a "recommended" modem for use with HylaFAX, per the Modems page.  Current bid is $0.50  (fifty cents).

 

Be sure to check the info on the modems page ( http://www.hylafax.org/Modems/index.php ) when shopping for a modem for HylaFAX.
