I guess by fax-destinations you mean different fax numbers? Yes we are
running this in a semi-live environment where faxes are being sent to
multiple destinations.

My config.faxCAPI is as follows

I have created a second file config.faxCAPI1. This seemed to spawn a
second c3faxsend process, but this process never successfully sent a
fax.

Looking forward to you suggestions...

Did you test with different fax-destinations? HylaFAX does send one fax 
per destination at the same time.

If you did and it didn't work, please post your config.faxCAPI.
