I hope you don't mind me answering this...

This happens when notify takes some time to complete running.

Currently, that blocks faxq, so that it doesn't get to processing FIFO
event's quickly enough.

There is a bug for it:
	http://bugs.hylafax.org/bugzilla/show_bug.cgi?id=445

It's in CVS head, and should be into 4.1 releases soon.

If you copy the whole lines from syslog (including timestamps), it would
be useful.
