The log you sent was for efax, not HylaFAX, but to answer your question, +FHS:11 means "no answer"... if the remote is answering, and if it is a fax machine, then there's probably some handshaking error that is tricking the modem into resulting in +FHS:11, but I'd need to see HylaFAX trace logs to know how to instruct you from there.
