The device is wrong, I think.  After you install the backend and model file, 
you should get 'Local Hylafax Server' in the comb box for 'Device'.

I noticed that for some reason the script "hylafax" that goes into the 
/usr/lib/cups/backend, isn't marked for execute in the release files.  I 
don't know if this matters, but it might.
