This is often due to people posting to the list before subscribing which sends the post to a moderator (necessary anti-spam measure). They don't read the reply, which says their post will eventually be sent, and just subscribe and post again. Sometimes they repeat this several times until they subscribe with the right email address to allow their posts to go through unmoderated. When the moderator approves the post later, you end up getting multiple, identical postings. So no malfunction as far as I can tell.
