I have the same config as you have with no problem :

the only differences with my config file is :
ModemResetCmds:         AT&Eyour-number
ModemResetCmds:         AT&L930          (for MSN)
ModemResetCmds:         ATS23=3          (for MSN)

here is solving somes problems of answering calls (I have noticed 
that you have not this parameter --> AT+FAA=1
what i have changed to solve this is :
ModemSetupAACmd:        AT+FAA=0 (instead of 1)

It seems to be an initialization problem, what happen when you 
restart hylafax ?
