It has nothing to do with the coverpage.  Notice that if you omit the
coverpage then the repeating page is the first one.  Notice that you
received RTN.  So HylaFAX does exactly what it was told to do: resend the
page over again.

My guess is that your modem is RTN inept.  See RTNHandlingMethod in 'man
hylafax-config'.
