I have the same problems with a USR USRobotics Sportster Voice 33600.
I've configured the modem as Class 1 and receiving of any facsimiles
works just fine, so I think the modem should not be defective. But if
I try to send a facsimile, I get the message:

If you need it, I can send the whole output from /var/log/messages
(about 150 lines).
As I read in other eMails, this problem should be solved in 4.1, but
unfortunately, I still get this error.
I use hylafax 4.1 (rpm) on a RedHat 7.0
Thanks,
Michael
