I just had a look at MIMEState.c++ and MIMEState.h
As far as I understand it, 
    Content-type: application/pdf; encoding=base64, charset=iso-8859-1
should work out of the box without any alteration. Unfortunately I
can't test it on my private  system, due to elderly Hylafax version. :-(
