Hmmm... we saw this problem before. Check the mail archives. It always happened when there was a busy signal and then the next call was always to the same number (that's abnormal - I get busy signals all the time here without that problem). If I remember right, it turned out to be a defective modem... but if it wasn't that it was fixed by a HylaFAX upgrade.
