Two things come instantly to mind:
Does your /var/spool/fax/etc/hosts{.hfaxd} contain 127.0.0.1?
Do you have something that might be firewalling connections from
localhost to port 4559? Try telnetting to localhost port 4559 and see
if you get a response.
But I think it's the first one.
