Phase D is the 300 bps post page interchange; it seems that you may have
had a premature carrier drop reported, but, as someone else said, the
trace level is inadequate.

Note that Rocket cards, whilst having high throughput, don't have good
latency as they poll, rather than use interrupts.  I can't remember whether
the poll is every 10 or 100ms with standard drivers.  I would check whether
the latter is compatible with class 1 fax timing constraints, although any
such problem would happen between pages, not a third of the way down one.
