Not trivially. You'd somehow have to merge the two images, which is
possible with some software (such as "merge" from the ImageMagick
software) or not use a separate cover page and include this stuff in
your Windows created Postscript file.

I don't know why you are getting a blank page: look carefully into your
printer settings for WHFC...
