flexfax: email notification reports success but fax has failed

We have hylafax 4.0pl2 and whfc 1.0.5.  A fax failed to go but an email to
the
sender reported fax completed OK.

email from uucp says:
Subject: facsimile job 210 to 0171 7530289 completed

Your facsimile job to 0171 7530289 was completed successfully.

in the syslog:
