flexfax: Hylafax not accepting fax

We are using Hylafax with a third party commerce application by Open Market
called Transact.  Transact uses Hylafax to send taken orders to merchants
who have stores in the Transact product.

Currently what I'm seeing is that Transact is creating the fax document and
it is sending it to Hylafax.  Hylafax is not accepting any faxes though.

This morning I placed a test order and saw that Transact accepted it and
sent it to the fax server.  In the syslog I saw these messages though:

Usually we can kill the fax processes and start them up and this fixes
whatever problem there was.  This morning when we did this, the fax server
still would not accept faxes.

When I run faxstat, I see:

$ faxstat
Server on localhost:FIFO for all modems: Running.
Server on localhost:ttyb for 687.7000: Running and idle.

I have tried sending faxes via the command line using the sendfax command
and Hylafax would not accept the fax either.  The log file in the 
/apps/omtx/back/tms-fax-s/hylafax/spool/fax/log directory hasn't changed
since Friday when our problems started to occur.

Where can I being to debug this problem?
