send problem

This one's easy.  The phone number is wrong, or the phone line is bad,
or the modem is broken:

Here the receiving fax machine/modem's telephone line was busy

Here the sending modem just refused to give a dial out response code
after 3 minutes.

Here something at the other end of the phone line answered, but initial
attempts to establish a modem connection failed (e.g., a person answered
the phone).
